ABSTRACT

Objective To establish a rational drug use model of PIVAS and promote the rational drug use in clinical practices by enhancing the quality of prescription review by pharmacists. Methods The PIVAS physician orders received from 2014 to 2021 were extracted through the hospital information management (HIS) system. The types of irrational physician orders were statistically analyzed, the improvements were made by the method of quality control circle (QCC). Results The model of PIVAS rational drug use formed a standardized process. The proficiency of physician order review was improved. From 2014 to 2021, the number and proportion of unreasonable physician orders in PIVAS decreased year by year. Every type of unreasonable physician orders was improved year by year. Conclusion The professional and technical levels of pharmacist for physician order review at our hospital were improved by the model of PIVAS rational drug use. The quality of pharmaceutical service was significantly improved which ensured the safety of patients' medication.

ABSTRACT

Based on mass spectrometry(MS)-guided separation strategy, compound 1 was obtained from the roots of Rhus chinensis. By comprehensive analysis of high resolution-electrospray ionization-mass spectrometry(HR-ESI-MS), nuclear magnetic resonance(NMR) data, and quantum chemical calculation of NMR(qcc-NMR) parameters, compound 1 was elucidated as rhuslactone, a 17-epi-dammarane triterpenoid with a rare 17α-side chain. An HPLC-ELSD method for its quantification in R. chinensis was established and adopted for the quantification of rhuslactone in different batches of R. chinensis. Rhuslactone displayed a good linear relationship within the range of 0.021 3-1.07 μmol·mL~(-1 )(r=0.997 6), and the average recovery was 99.34% [relative standard deviation(RSD) 2.9%). Moreover, the results of the evaluation test of the preventive effects of rhusalctone on coronary heart disease(CHD) and thrombosis showed that rhuslactone(0.11 nmol·mL~(-1)) significantly alleviated heart enlargement and venous congestion and increased cardiac output(CO), blood flow velocity(BFV), and heart rate, thereby reducing thrombus formation in zebrafish with CHD. The effects of rhuslactone on CO and BFV were superior to that of digoxin(1.02 nmol·mL~(-1)), and its effect on improving heart rate was comparable to that of digoxin. This study provides experimental references for the isolation, identification, quality control, and application of rhuslactone from R. chinensis against CHD. It is worth mentioning that this study has discussed some omissions in the determination of the stereochemistry of C-17 in dammarane triterpenoids in the present coursebook Chemistry of Chinese Medicine and some research papers, that is, the compound may be 17-epi-dammarane triterpenoid. This paper has also proposed steps for the establishment of C-17 stereochemistry.

ABSTRACT

To explore the effect and experience of quality control circle(QCC) in quality control testing for CT scanners, the quality control circle group was set up to determine the theme of quality control circle, and the causes of the failure of the quality control testing for CT scanners were analyzed, then the corresponding corrective measures were formulated and carried out. After the activity of the quality control circle, the qualified rate of CT quality control testing in the second level 2nd Class of public hospitals and private hospitals in Shanghai increased from 40.6% to 86.1%. By conducting quality control circle activities, we found the problems existed in the quality control testing of CT scanners, and put forward many corresponding corrective measures and solutions which finally improved the qualified rate of CT quality control testing.

ABSTRACT

To study the role of the activity in reducing the incidence of water leakage in digestive endoscopy, through the activity of the quality control circle (QCC), the reasons for water leakage and high maintenance cost are analyzed deeply. Then, the author formulated and implemented targeted measures to intervene in the non-standard aspects of daily management of endoscopy. Then targeted measures has been taken to improve the nonstandard daily management. Results demonstrate that the application of the quality control circle can significantly reduce the incidence of water leakage in the digestive endoscope and control the other faults caused by the leakage. This not only reduces the high maintenance cost of endoscopy, improves the utilization rate and turnover rate of endoscopy, but also fully explores personal potential, enhances team cohesion, and improves quality management awareness.

ABSTRACT

Through establishing QCC,the theme is confirmed, the plan is made, and the data is analyzed to explore QCC on improving the nursing unit of tumor patients'pain.(1)Visible result shows that, through the activities of QCC,the pain assessment accuracy is markedly increased from 70.48% to 90.48%.The difference is statistically significant (P<0.05).(2)Invisible result shows that QCC members mark themselves based on QCC grasp degree,team-work ability, professional knowledge, communication coordinate ability, confidence, responsibility and enthusiasm before and after QCC.

ABSTRACT

Objective To enhance oral tablets ward pharmacy inventory efficiency and improve the comprehensive quality of the pharmacist by QCC activities .Method Main factors to extend the ward pharmacy tablet counting time were analyzed through collecting the previous ward pharmacy tablet count time and account matching rate .According to the main problems , the strategies were found out to be implemented ,identify and evaluate the implementation result .Result In view of the oral medicine ward pharmacy inventory of the reasons for the low efficiency ,following measures should be taken :reducing oral drug dispensing station varieties ,finishing cargo ,strengthening personnel training ,changing inventory counting method and optimi-zation of inventory materials and re-enacted drugs location code and so on .Ward pharmacy tablet counting time reduce from 6 .5 hours to 3 hours (reducing by 53 .85% ) .Conclusion The QCC activities could significantly improve the oral medicine ward pharmacy inventory efficiency and comprehensive quality of pharmacists .

ABSTRACT

OBJECTIVE:To reduce the error rate of inventory in the automated pharmacy of our hospital. METHODS:Activi-ties were designed and implemented by the management method of quality control circles(QCC)-PDCA(Plan,Do,Check and Ac-tion)cycle. The reasons for the errors of inventory in the automated pharmacy were analyzed to investigate and implement counter-measures. Visible and invisible achievements were evaluated, and then standardized processes were made. RESULTS:The errors of inventory in the automated pharmacy mainly included those of drug dispensing, drug shelving, drug return sheet, automatic medi-cine dispensing machine and system. In view of the above reasons, relevant standards were formulated and performed, including the process of warehouse-out check and shelving of drugs, drug dispensing process for the automated pharmacy, the process of sec-ondary check, etc. After the implementation of the activities, the error rate of inventory in the automated pharmacy reduced from 9.17% to 3.77%,which was visible achievement;and the above-mentioned standardized processes could ensure continuous run-ning of PDCA cycle. The practice, sense of responsibility, communication and coordination of management members in PDCA cy-cle namely invisible achievements were improved to some extent. CONCLUSIONS:The management method of QCC-PDCA cycle is feasible in reduction of the error rate of inventory in the automated pharmacy, which can provide a reference for automated phar-macy management.

ABSTRACT

Objective To investigate the effect of implementation of QCC (quality control circle) in management of preoperative examination. Methods Through analysis of the common factors of patients' preoperative examination, the QCC was used to manage every parts of preoperative examination, and the nursing process of preoperative examination for resident patients was formulated. The effect of nursing management of preoperative examination before and after the implementation of QCC was investigated and compared. Results The result showed that the degree of satisfaction was significantly higher than before, and the risk incident was significantly lower than before. Conclusions To carry out QCC could improve the efficiency of preoperative examination and the patients' safety,as well as enhance the awareness of the nursing management.
